Become a community partner

This takes about 5 minutes, and there's nothing to upload. We'll ask for your logo and photos after we talk. Everything is required unless it's marked optional.

1About you

The basics, so we know who we're talking with.

For example, yoursite.com or facebook.com/yourpage.
We'll never publish a home address.
We'll send your confirmation here.
We'll use this to set up a short call.
A few sentences is plenty.
If someone referred you, we'll ask who on our call so we can thank them.

2What you offer

Plain words are perfect.

Please leave out health results or success rates. Partner pages never include them.
Who do you serve?Check all that apply.
How do you deliver your services?Check all that apply.
How do people pay for your services?Check all that apply. If we list these, we'll add that coverage depends on each person's plan and they should confirm with you before booking.
Do you help veterans prepare or file VA benefits claims?VA requires accreditation to help with VA claims, and no one may charge for an initial claim.

3Your free first step

Every community partner offers a free first step to the people we serve. It's the heart of your partner page.

For example, a first visit, an assessment or a class. Tell us what's included and any limits. It must be truly free, with no purchase required.
After the free first step, what else do you offer? So people know what to expect. Paid services are completely fine.
How soon will you reach out to someone who signs up through your page?Choose a time you can meet every time.

4Licenses, insurance and ties to OWH

Honest answers help us describe your work accurately and keep everyone protected. A yes here usually just means we'll talk it through on our call.

Do any of your services require a license or certification? For example, a medical, dental, counseling or massage license.
We look up licenses on the board's public website and check federal exclusion lists as part of our review. We'll ask about license numbers and the rest of your team on our call.
In the past 5 years, has your organization, or anyone who would serve our community, had a license disciplined, been excluded from a federal health program, or received an FDA or FTC warning letter? No single answer here rules you out.
What liability insurance do you carry?Partners who provide in-person or hands-on services need general liability, and licensed providers also need professional liability. We'll ask for your certificate later.
Does anyone at your organization have a personal, family or business relationship with OWH, its founder, staff or board?Saying yes is completely fine. We'll talk it through on our call.
Does anything of value flow between your organization and OWH, its founder, staff or board, in either direction?Check all that apply. Partnership is free. OWH never pays or accepts anything of value for a listing, a placement or a referral, and giving to OWH never affects whether or how a partner is listed.
For example, a family member of a board member, a current vendor or a planned donation.

5Partner standards and signature

Every new partner agrees to the same seven standards. They protect the people we serve, and they protect you too.

  1. A truly free first step: no purchase, no pressure, and within the health-care laws on offers to Medicare, Medicaid and TRICARE beneficiaries.
  2. Plain words, no promises: no promised results, success rates or suicide-prevention claims.
  3. Licensed and insured: licensed wherever a license applies, and insured for hands-on services.
  4. Safety and scope: stay within your training, use only VA-accredited help for VA claims, and connect anyone in crisis to 988, then press 1.
  5. Respect and privacy: contact every signup on time, and never sell or share their information.
  6. Fair to everyone: serve people without discrimination, welcome service animals and never require religious activity.
  7. Honest, separate and nonpartisan: keep your details current, use OWH's name only as approved, and know that either of us may end the partnership at any time.

Your privacy: only OWH's founder and staff review requests. We use your answers only to review your request and contact you, and we never sell or share them. If we don't move forward, we'll delete your request whenever you ask.

Your typed name is your signature. We'll record the date and time you submit and the version of the standards you agreed to.

Michael Andrew Jones, our founder, reads every request himself. You'll get a confirmation email right away.

Operation Whole Health is a Nevada 501(c)(3) nonprofit (EIN 41-4602379). We certify, endorse and accredit no one. Submitting a request isn't an approval, and nothing about your organization goes live until you approve it in writing. In crisis? Call 988, then press 1, or text 838255.
